4. Documents Required at the time of DV
Candidates must produce the original documents along with two sets of self-attested photocopies:
- Copy of Online Application Form.
- Two Passport size recent color photographs.
- Valid Photo ID proof (Aadhaar, Voter ID, PAN).
- Date of Birth Certificate (10th Diploma / Marks card).
- Degree / Diploma Certificate in Prosthetics and Orthotics from a recognized institute.
- Registration Certificate from the Rehabilitation Council of India (RCI).
- Valid Domicile Certificate of J&K UT.
- Category Certificate, if applicable.
